Sports physical form now online at District website

Thanks to a parent's suggestion we have put the Pre-participation Sport Examination form online at our District website.

The form is under the Athletics tab on the home page included in the listing of sports at the top of the information under that tab.

The form is in a pdf format and can be filled out on your computer and then copied.
